Synopsis: AS A RESULT OF ITS CONTINUING INVESTIGATION OF THE CRASH OF AIR FLORIDA FLIGHT 90, A BOEING 737-222 (N62AF) ON JANUARY 13, 1982, THE NATIONAL TRANSPORTATION SAFETY BOARD BELIEVES THAT IMMEDIATE CORRECTIVE ACTION IS NEEDED IN THE AREA OF COLD-WEATHER OPERATIONS PROCEDURES. THE AIRCRAFT HAD DEPARTED FROM RUNWAY 36 AT WASHINGTON NATIONAL AIRPORT IN MODERATE TO HEAVY SNOWFALL AND LOW VISIBILITY. THE AIRCRAFT FAILED TO ACHIEVE A SUFFICIENT RATE OF CLIMB, STRUCK THE 14TH STREET BRIDGE ABOUT 4,500 FEET FROM THE DEPARTURE END OF THE RUNWAY, AND CRASHED INTO OF THE POTOMAC RIVER. SEVENTY-FOUR OF THE 79 PERSONS ABOARD THE AIRCRAFT WERE KILLED EITHER ON IMPACT OR BY DROWNING, AND 4 PERSONS IN AUTOMOBILES ON THE BRIDGE WERE KILLED WHEN THE VEHICLES WERE STRUCK BY THE DESCENDING AIRCRAFT.
Recommendation: THE NTSB RECOMMENDS THAT THE FEDERAL AVIATION ADMINISTRATION: IMMEDIATELY NOTIFY ALL AIR CARRIER OPERATORS OF THE POTENTIAL HAZARD ASSOCIATED WITH ENGINE INLET PRESSURE PROBE ICING, AND REQUIRE THAT THEY PROVIDE FLIGHTCREWS WITH INFORMATION ON HOW TO RECOGNIZE THIS HAZARD AND REQUIRING THAT FLIGHTCREWS CROSS-CHECK ALL ENGINE INSTRUMENTS DURING THE APPLICATION OF TAKEOFF POWER. Response: Your March 11, 1982, action to issued Air Carrier Operations Bulletin No. 7-82-2, Cold Weather Procedures, satisfies the intent of A-82-6. This bulletin emphasizes the problems associated with engine inlet icing and provides operational guidance concerning use of engine anti-ice, cross-checking engine parameters during takeoff and training to recognize abnormal engine performance indications. A-82-6 has been classified as "Closed--Acceptable Action."

THIS ACOB EMPHASIZES THE PROBLEMS ASSOCIATED WITH ENGINE INLET ICING AND SUGGESTED OPERATIONAL PROCEDURES.

Response: The Safety Board is pleased to note the FAA's expeditious processing and dissemination of the verbatim contents of the safety recommendation letter to all air carriers, manufacturers, foreign authorities and other interested groups. We also note the January 29, 1982, telephone conference call in which FAA headquarters staff advised FAA regional flight standards division personnel to contact all air carriers to review the NTSB recommendations and to review each carrier's cold weather operational procedures, training programs and operations manuals. We have the following comments regarding FAA actions in response to our recommendations. The FAA has reviewed, with all air carriers, the potential hazards associated with engine inlet pressure probe icing, the requirement that flight crews know how to recognize this hazard, and that flightcrews cross-check all engine instruments during the application of takeoff power. The FAA also plans to issue an Air Carrier Operations Bulletin concerning the appropriate use of engine anti-ice. These Data Source: NTSB Recommendations to FAA and FAA Responses actions satisfy the intent of A-82-6, which will be classified as "Open--Acceptable Action" pending receipt of the ACOB.

IN ADDITION, FAA FLIGHT STANDARDS PERSONNEL CONTACTED ALL AIR CARRIERS AND REVIEWED THIS RECOMMENDATION. FINALLY, AN AIR CARRIER OPERATIONS BULLETIN THAT EMPHASIZED THE APPR PRIATE USE OF ENGINE ANTI-ICE WILL BE ISSUED TO AIRCREWS IN THE NEAR FUTURE.
