Details
Synopsis: ON MAY 30, 1970, A TANK TRUCK PARTIALLY FILLED WITH LIQUEFIED EXPLODED, WITHOUT WARNING, AFTER MAKING A DELIVERY AT THE VICTORY MEMORIAL HOSPITAL IN BROOKLYN, NEW YORK. THE FORCE OF THE EXPLOSION AND ENSUING FIRES RESULTED IN FATAL INJURIES TO THE DRIVER AND A BYSTANDER, MINOR INJURIES TO 30 OTHER PERSONS, AND SUBSTANTIAL PROPERTY DAMAGES.
Recommendation: THE NTSB RECOMMENDS THAT THE COMPRESSED GAS ASSOCIATION, INC.: SPONSOR THE ACCELERATED DEVELOPMENT OF QUANTIFIED, ENFORCEABLE INDUSTRY CLEANLINESS STANDARDS OR SPECIFICATIONS FOR OXYGEN CARGO TANKS, APPLICABLE OVER THE LIFE CYCLE OF THE EQUIPMENT, AND THE RESEARCH FOR AND DEVELOPMENT OF COMPARATIVE RANKINGS FOR THE RELATIVE SUSCEPTIBILITY OF ALTERNATIVE MATERIALS OF CONSTRUCTION TO INITIATION AND SPREADING OF INDUCED REACTIONS WITH OXYGEN IN CARGO TANKS TRANSPORTING OXYGEN.

Response: COMPRESSED GAS ASSOCIATION, INC. 1. THE COMPRESSED GAS ASSOCIATION AT ITS ANNUAL SUMMER MEETING AT ABSECON, NEW JERSEY, ON JUNE 14-15, VOTED TO ESTABLISH A TASK GROUP TO PREPARE A PROGRAM FOR THE RESEARCH RECOMMENDED BY THE NTSB IN THE SUBJECT REPORT. IT HAS ALREADY RECEIVED ONE OFFER FROM A FOREIGN METAL SUPPLIER TO PARTICIPATE IN AND CONTRIBUTE FUNDS TO SUCH A PROGRAM. THE ALUMINUM ASSOCIATION AND TRUCK MANUFACTURERS ALSO OFFERED TO PARTICIPATE (RECOMMENDATION 5.). 2. ASSOCIATION STAFF MEMBERS ARE PLANNING TO COMMUNICATE WITH THE DEPARTMENT OF TRANSPORTATION TO EXPEDITE THE DEVELOPMENT AND PUBLICATION OF CARGO TANK SPECIFICATIONS IN THE REGULATIONS (RECOMMENDATION 4.). 3. COPIES OF REPORT DISTRIBUTED TO ALL MEMBERS OF A TASK FORCE FORMED FOR THE PURPOSE OF DEVELOPMENT OF SPECIFIC PROCEDURES FOR CLEANING EQUIPMENT FOR OXYGEN SERVICE. SEPTEMBER 9, 1971 COMPRESSED GAS ASSOCIATION, INC. (CGA) WHEN THE NATIONAL BUREAU OF STANDARDS TEST RESULTS (AT BOULDER FOR DOT, SEE RECOMMENDATION BSTS-H-71-44, PAGE 44) ARE AVAILABLE, CGA WOULD LIKE TO REVIEW SAME WITH NBS, DOT, AND NTSB IN ORDER TO REACH A MUTUAL UNDERSTANDING AS TO WHAT HAS TO BE DONE. A PROGRAM FOR PHYSICALLY ACCOMPLISHING THE WORK COULD THEN BE ESTABLISHED. JANUARY 26, 1972 ASSISTANT SECRETARY FOR SAFETY AND CONSUMER AFFAIRS, DOT LETTER STATES THAT: THIS TYPE OF EXCHANGE AND MUTUAL EFFORT HAS ALWAYS BEEN DESIRED AND IS A CONTINUING EFFORT. SOME EXAMPLES ARE HAZARDOUS MATERIALS SCHOOLS CO-SPONSORED BY DOT AND VARIOUS UNIVERSITIES, COOPERATION WITH STATE AGENCIES ON INSPECTION TRIPS, STATE AND REGULATORY GROUP MEETINGS, COOPERATION WITH THE COUNCIL OF STATE GOVERNMENTS, FIELD STAFF CONTACT WITH STATE AGENCIES, THE CITIZEN ADVISORY GROUP AND FHWA COOPERATIVE AGREEMENTS WITH EVERY STATE. CONTACT IS ALSO MADE WITH EACH STATE GOVERNOR ON SELECTED PROJECTS.
