Synopsis: ABOUT 7:04 P.M., ON AUGUST 20, 1976, A TRACTOR-SEMITRAILER DESCENDING A STEEP GRADE IN VALLEY VIEW, OHIO, COLLIDED WITH 10 AUTOMOBILES THAT WERE STOPPED AT A SIGNALIZED INTERSECTION NEAR THE BOTTOM OF THE GRADE. FIRE ENSUED. EIGHT OF THE 27 AUTOMOBILE OCCUPANTS DIED AND 15 WERE INJURED; THE TRUCKDRIVER SUSTAINED MINOR INJURIES. THE NATIONAL TRANSPORTATION SAFETY BOARD DETERMINES THAT THE PROBABLE CAUSE OF THIS ACCIDENT WAS THE INABILITY OF THE IMPROPERLY ADJUSTED AND PARTIALLY INOPERATIVE SERVICE BRAKE SYSTEM ON THE TRACTOR-SEMITRAILER TO ADEQUATELY SLOW THE VEHICLE AS IT DESCENDED THE GRADE. INADEQUATE PRETRIP INSPECTION BY THE DRIVER AND THE LACK OF REQUIRED MAINTENANCE AND INSPECTION BY THE CARRIER FAILED TO IDENTIFY AND TO CORRECT THE UNSAFE CONDITION OF THE BRAKE SYSTEMS. THE FAILURE OF THE ROAD SIGNS TO PROVIDE ADVANCE WARNING INFORMATION CONCERNING THE LENGTH AND STEEPNESS OF THE GRADE AND THE PRESENCE OF THE SIGNALIZED INTERSECTION, PREVENTED THE DRIVER FROM TAKING EARLY EVASIVE ACTION.
Recommendation: THE NTSB RECOMMENDS THAT THE STATE OF OHIO: CONDUCT, IN COOPERATION WITH THE CITY OF GARFIELD HEIGHTS AND VILLAGE OF VALLEY VIEW, A TRAFFIC SURVEY AND ANALYSIS OF THAT PART OF STATE ROUTE 17 (GRANGER ROAD), EXTENDING FROM THE APPROACH TO THE HILL CREST IN GARFIELD HEIGHTS TO CANAL ROAD IN VALLEY VIEW, TO DETERMINE AND INSTALL NEEDED ADVANCE WARNING SIGNS WHICH WILL PROVIDE DRIVERS OF ALL TYPES OF VEHICLES INFORMATION RELATING TO THELENGTH AND STEEPNESS OF THE GRADE AND THE PRESENCE OF THE SIGNALIZED INTERSECTION.

Response: OHIO DOT TOOK THE FOLLOWING ACTIONS: 1. A SPECIAL TRUCK ROUTE WAS INITIATED TO DIVERT TRUCK TRAFFIC FROM THAT PART OF ROUTE 17 WHERE THE ACCIDENT TOOK PLACE. 2. A JOINT LOCAL AND STATE TRAFFIC ENGINEERING SURVEY WAS MADE AND SIGNING IMPROVEMENTS WERE OR WILL BE MADE: A. SYMBOLIC TRUCK PROHIBITION SIGNS WERE FURNISHED TO GARFIELD HEIGHTS AND VALLEY VIEW FOR POSTING ON ROUTE 17. B. A SYMBOLIC 48" X 48"SIGNAL AHEAD SIGN WAS PLACED ON ROUTE 17 IN ADVANCE OF THESIGNAL AT CANAL ROAD. C. AN ADDITIONAL SYMBOLIC HILL SIGNWITH A "1000 FT. AHEAD" PLAQUE, PLUS A PLAQUE ON THE EXISTING HILL SIGN INDICATING A 10% GRADE, 3/4 MILE HAVE BEEN INSTALLED ON ROUTE 17. 3. THE TWO MUNICIPALITIES WERE ADVISED OF THE REQUIREMENTS AND PROCEDURES FOR FEDERAL FUNDED HIGHWAY SAFETY PROJECTS.
