Synopsis: ON APRIL 5, 1987, TWO OF THE FIVE SPANS OF THE NEW YORK STATE THRUWAY (I-90) BRIDGE OVER THE SCHOHARIE CREEK FELL ABOUT 80 FEET INTO A RAIN-SWOLLEN CREEK AFTER PIER 3, WHICH PARTIALLY SUPPORTED THE TWO SPANS, COLLAPSED. NINETY MINUTES AFTER THE INITIAL COLLAPSE, PIER 2 AND A THIRD SPAN COLLAPSED. FOUR PASSENGER CARS AND ONE TRACTOR-SEMITRAILER PLUNGED INTO THE CREEK, AND 10 PERSONS WERE FATALLY INJURED.
Recommendation: THE NTSB RECOMMENDS THAT THE AMERICAN ASSOCIATION OF STATE HIGHWAY AND TRANSPORTATION OFFICIALS: PROVIDE SPECIFIC GUIDANCE ON MAINTENANCE OF FOUNDATIONS OF BRIDGES THAT ARE DEPENDENT UPON RIPRAP FOR SCOUR PROTECTION TO SPECIFY THAT FOLLOWING EACH INSPECTION THE RIPRAP BE REPLENISHED TO MEET THE DESIGN SPECIFICATIONS.

Response: Safety Recommendation H-88-14 asked the AASHTO to provide specific guidance on maintenance of foundations of bridges that are dependent upon riprap for scour protection to specify that following each inspection the riprap be replenished to meet the design specifications. The Safety Board understands that the AASHTO Subcommittee on Maintenance has issued a new page 160 to the Manual for Bridqe Maintenance. According to paragraph two of page 160, a thorough scour evaluation must be conducted in order to adequately evaluate the size of stone required at bridge crossings. Response: BASED UPON THE RECOMMENDATIONS OF THE SUBCOMMITTEE, WE HAVE ISSUED A NEW PAGE 160 TO THE "MANUAL" WHICH ELIMINATES THE CHART AND RECOMMENDS A THOROUGH SCOUR EVALUATION. THIS CHANGE WAS BALLOTED THROUGH THE SUBCOMMITTEE, THE STANDING COMMITTEE ON HIGHWAYS AND THE EXECUTIVE COMMITTEE. THE NEW PAGE HAS BEEN SENT TO ALL MEMBER DEPTS. AND HAS BEEN INSERTED IN ALL THE "MANUALS" IN OUR INVENTORY.
