Synopsis: ABOUT 6:45 A.M., CENTRAL STANDARD TIME, ON NOVEMBER 19, 1988, A GREYHOUND BUS WITH 45 OCCUPANTS, TRAVELING SOUTH BOUND THROUGH A CONSTRUCTION ZONE ON INTERSTATE HIGHWAY 65 IN NASHVILLE, TENNESSEE, SUDDENLY WENT OUT OF CONTROL DURING A STEERING MANEUVER, ROTATED 190 DEGREES CLOCKWISE IN THE SOUTHBOUND LANES, OVERTURNED ON ITS LEFT SIDE, AND CAME TO REST FACING NORTHBOUND ON THE SOUTHBOUND EMBANKMENT. THE UNRESTRAINED BUS DRIVER AND 38 PASSENGERS WERE INJURED IN THE ACCIDENT. TWELVE PASSENGERS SUSTAINED SERIOUS INJURIES, AND THE BUS DRIVER AND 26 PASSENGERS RECEIVED MINOR INJURIES. SIX PASSENGERS WERE NOT INJURED.
Recommendation: THE NTSB RECOMMENDS THAT GREYHOUND LINES, INC.: ESTABLISH AND ENFORCE A POLICY THAT EACH INDIVIDUAL CONDUCTING THE FEDERALLY REQUIRED ANNUAL REVIEW OF A BUS DRIVER'S TRAFFIC RECORD BE A SUPERVISOR WHO IS FAMILIAR WITH THAT RECORD, QUALIFIED TO INTERPRET IT, AND AUTHORIZED TO IMPOSE APPROPRIATE MEASURES IN RESPONSE TO FINDINGS FROM THE REVIEW.

Response: Safety Recommendation H-89-29 requested Greyhound to establish and enforce a policy that each individual conducting the federally required annual review of a busdriver's traffic record be a supervisor who is familiar with that record and qualified to interpret it. The Safety Board understands that the annual review procedures for driver qualification files have been modified to require the review to be conducted under the direct supervision of the safety and manpower managers. Furthermore, we have examined the driver qualification file review form. Because a new driver review policy has been established, Safety Recommendation H-89-29 has been classified as "Closed--Acceptable Action."

Response: ANNUAL REVIEW PROCEDURES FOR DRIVER QUALIFICATION FILES HAVE BEEN MODIFIED AND REQUIRE THE REVIEW TO BE CONDUCTED UNDER THE DIRECT SUPERVISION OF OUR SAFETY AND MANPOWER MANAGERS. A COPY OF THE REVIEW FORM WHICH IS ATTACHED TO DRIVER QUALIFICATION FILES IS ENCLOSED. WE BELIEVE THAT BY HAVING THE MANAGEMENT OF THE DRIVER QUALIFICATION FILES UNDER THE DIRECT SUPERVISION OF OUR SAFETY AND MANPOWER MANAGERS WE ARE MEETING THE REQUIREMENT RECOMMENDATION H-89-29.
