Synopsis: ABOUT 3:25 P.M. A 1988 MACK TRUCK WITH A CONCRETE MIXER BODY WAS UNABLE TO STOP AS IT APPROACHED A "T" INTERSECTION AT THE BOTTOM OF AN EXIT RAMP IN PLYMOUTH MEETING, PENNSYLVANIA. AS THE TRUCK PROCEEDED THROUGH THE INTERSECTION, IT COLLIDED WITH & OVERRODE A 1985 SUBARU PASSENGER CAR. THE SUBARU DRIVER WAS KILLED; THE TRUCKDRIVER SUSTAINED MINOR INJURIES. THE TRUCKDRIVER WAS UNRESTRAINED; THE SUBARU DRIVER WAS FOUND RESTRAINED IN HER VEHICLE. THE WEATHER WAS CLEAR & DRY. NO FIRE ENSUED, & NO OTHER VEHICLE OCCUPANTS WERE INVOLVED IN THE ACCIDENT.
Recommendation: THE NTSB RECOMMENDS THAT THE JDM MATERIALS COMPANY, INC.: IMPLEMENT PROCEDURES THAT WILL ENSURE THAT AIR BRAKE MANUFACTURERS' SUGGESTED INSTALLATION, INSPECTION, & FUNCTIONAL TEST PROCEDURES ARE ADHERED TO BY COMPANY MECHANICS PERFORMING MAINTENANCE ON THE COMPANY'S TRUCK BRAKE SYSTEMS.

Response: The Safety Board notes that JDM instituted several safety procedures, including a prompt post-accident inspection of the entire fleet, following the accident. Additionally, as a result of your discussions with Mr. O’Neill of our staff, JDM mechanics are now required to adhere to air brake manufacturers’ procedures when installing, testing, and inspecting company truck brake systems. Based on this information, Safety Recommendation H-97-35 is classified “Closed--Acceptable Action.”

Response: Letter Mail Controlled 03/26/2001 7:28:09 PM MC# 2010253 As stated in Mr. Hall’s letter, a prompt post accident inspection of the entire fleet was completed to verify the status of all the air brake systems. As a follow up to that inspection, each and every repair that may even remotely affect the air brake systems triggers a complete examination of those systems. If a vehicle has not had any repair, which would trigger an examination, J.D.M. Materials Co. has made an examination of the air brake systems concurrent with the semi-annual Pennsylvania state inspection program.
