Synopsis: ABOUT 3:25 P.M. A 1988 MACK TRUCK WITH A CONCRETE MIXER BODY WAS UNABLE TO STOP AS IT APPROACHED A "T" INTERSECTION AT THE BOTTOM OF AN EXIT RAMP IN PLYMOUTH MEETING, PENNSYLVANIA. AS THE TRUCK PROCEEDED THROUGH THE INTERSECTION, IT COLLIDED WITH & OVERRODE A 1985 SUBARU PASSENGER CAR. THE SUBARU DRIVER WAS KILLED; THE TRUCKDRIVER SUSTAINED MINOR INJURIES. THE TRUCKDRIVER WAS UNRESTRAINED; THE SUBARU DRIVER WAS FOUND RESTRAINED IN HER VEHICLE. THE WEATHER WAS CLEAR & DRY. NO FIRE ENSUED, & NO OTHER VEHICLE OCCUPANTS WERE INVOLVED IN THE ACCIDENT.
Recommendation: THE NTSB RECOMMENDS THAT THE PENNSYLVANIA DEPARTMENT OF TRANSPORTATION: DEVELOP AN EASILY ADMINISTERED INSPECTION PROTOCOL FOR DETECTING REVERSED AIR LINES & INOPERATIVE LOW-AIR-PRESSURE WARNING SWITCHES ON DUAL AIR BRAKE SYSTEM VEHICLES.

Response: THE SAFETY BOARD NOTES THAT THE PADOT HAS DEVELOPED AND WILL DISTRIBUTE A SAFETY ADVISORY TO ALL OFFICIAL PENNSYLVANIA INSPECTION STATIONS ALERTING THEIR MECHANICS TO THE NEED TO FOLLOW THE MANUFACTURER'S TESTING PROCEDURES FOR ALL BRAKE SYSTEM ITEMS THEY HAVE REPAIRED, REPLACED, OR INSPECTED. ADDITIONALLY, THE BOARD UNDERSTANDS THAT THE SAFETY INFORMATION WILL BE DISTRIBUTED TO ALL FACILITIES THAT OFFER THE PADOT'S MECHANIC CERTIFICATION COURSES AND THAT, SHOULD A STANDARDIZED INSPECTION PROTOCOL BE DEVELOPED FOR ALL BRAKE SYSTEMS, THE PADOT WILL CONSIDER INCORPORATING SUCH SYSTEMS INTO ITS INSPECTION PROGRAM REGULATIONS. BASED ON THIS INFORMATION, H-97-36 IS CLASSIFIED "CLOSED--ACCEPTABLE ALTERNATE ACTION."

Response: THE PADOT HAD DEVELOPED THE ENCLOSED ADVISORY BULLETIN TO INFORM INSPECTION STATIONS OF THE NEED TO FOLLOW THE MANUFACTURER'S TESTING PROCEDURES FOR ALL BRAKE SYSTEM ITEMS THEY HAVE REPAIRED, REPLACED, OR INSPECTED. THIS BULLETIN WILL BE DISTRIBUTED TO ALL OFFICIAL INSPECTION STATIONS IN PENN. AND ALL TRAINING FACILITIES AUTHORIZED TO OFFER THE SAFETY INSPECTION MECHANIC CERTIFICATION COURSE. IN THE FUTURE, SHOULD A STANDARDIZED INSPECTION PROTOCOL BE DEVELOPED, ADDRESSING AIR BRAKE SYSTEMS, THE DEPT. WILL CONSIDER INCORPORATING THEM INTO OUR INSPECTION PROGRAM REGULATIONS.
