Synopsis: ABOUT 11:30 A.M., ON NOVEMBER 30, 1988, A TRACTOR-FLATBED SEMITRAILER OPERATED BY HY YIELD BROMINE COMPANY OVERTURNED AT THE INTERSECTION OF TWO FARM ROADS IN A SPARSELY POPULATED AREA OF COLLIER COUNTY, FLORIDA. THE SEMITRAILER WAS LOADED WITH 32 CYLINDERS OF A POISONOUS AND TOXIC BY INHALATION MIXTURE, 98 PERCENT METHYL BROMIDE AND 2 PERCENT CHLOROPICRIN. ELEVEN OF THE CYLINDERS WERE FULL, EACH CONTAINING ABOUT 1,500 POUNDS OF THE POISONOUS MIXTURE, AND THE REMAINDER OF THE CYLINDERS WERE PARTIALLY FULL OR EMPTY EXCEPT FOR RESIDUE. THE DRIVER HAD COMPLETED THE SECOND OF FOUR SCHEDULED STOPS WHEN THE ACCIDENT OCCURRED.
Recommendation: THE NTSB RECOMMENDS THAT THE COLLIER COUNTY, FLORIDA. DETERMINE LOCATIONS WITHIN THE COUNTY IN WHICH EMERGENCY RESPONSE PERSONNEL ARE UNABLE TO COMMUNICATE WITH COUNTY DISPATCHERS BY RADIO, AND CORRECT COMMUNICATION DEFICIENCIES IDENTIFIED TO PROVIDE ADEQUATE EMERGENCY RESPONSE COMMUNICATIONS.

Response: Notation 5022C adopted by Board. Through correspondence with the Florida Highway Patrol, the Safety Board learned that in early 1991 a new statewide 800 MHz radio system was implemented and that the system "will include mutual aid radio channels for interagency coordination, including county emergency response." As a result, Safety Recommendation I-90-16 was classified "Closed-Acceptable Action," and staff believes that it is reasonable to now so classify I-90-13. Staff did not learn, however, through reviewing the correspondence of other recipients, whether the intent of I-90-14 or -15 was ever met. Consequently, staff proposes that the appropriate status for these two recommendations is "Closed-Unacceptable Action/No Response Received." Staff further believes that a letter to the CCBC at this time advising of this status change would serve no useful purpose and would be of little meaning to the CCBC.

Response: THE SAFETY BOARD HAS RECEIVED NO REPLY FROM THE COLLIER COUNTY BOARD OF COMMISSIONERS REGARDING I-90-13 THROUGH -15. THE SAFETY BOARD WOULD APPRECIATE LEARNING WHAT ACTION THE COLLIER COUNTY BOARD OF COMMISSIONERS HAS TAKEN OR INTENDS TO TAKE TO IMPLEMENT THESE RECOMMENDATIONS.
