Synopsis: ON AUGUST 7, 1992, THE UNITED KINGDOM PASSENGER VESSEL RMS (ROYAL MAIL SHIP) QUEEN ELIZABETH 2 (QE2) WAS OUTBOUND IN VINEYARD SOUND, MASSACHUSETTS, WHEN THE VESSEL GROUNDED ABOUT 2 1/2 MILES SOUTH OF CUTTYHUNK ISLAND. NO INJURIES OR DEATHS RESULTED FROM THIS ACCIDENT. HOWEVER, DAMAGE WAS SIGNIFICANT; TEMPORARY AND PERMANENT REPAIRS COST ABOUT $13.2 MILLION. IN ADDITION, THE TOTAL REVENUE LOST FOR THE PERIOD BEFORE THE VESSEL RETURNED TO SERVICE ON OCTOBER 2, 1992, WAS ESTIMATED AT $50 MILLION.
Recommendation: THE NTSB RECOMMENDS THAT THE NATIONAL OCEANIC AND ATMOSPHERIC ADMINISTRATION: INCLUDE INFORMATION ON U.S. COASTAL CHARTS AND IN U.S. COAST PILOT VOLUMES CONCERNING DEPT SURVEY DATES AND BRIEF DESCRIPTIONS OF SURVEY METHODOLOGY, INCLUDING SUCH ITEMS AS SURVEY TRACKLINE SEPARATION, SOUNDING METHOD, AND SOUNDING ACCURACY FOR PILOTAGE WATERS.

Response: THE BOARD IS PLEASED THAT NOAA HAS PUBLISHED CARTOGRAPHIC DIRECTIVE 002/93, WHICH REVISED THE NAUTICAL CHART MANUAL, VOLUME 1, SEVENTH (1992) EDITION, AND SPECIFIES THE PLACEMENT OF SOURCE DIAGRAMS ON ALL CHARTS 1:500,000 SCALE OR LARGER. IN ADDITION, AN EXPLANATORY TEXT AND MARTRIX REGARDING THE ERAS OF HYDROGRAPHIC SURVEY TECHNOLOGY HAVE BEEN FINALIZED AND WILL BE PUBLISHED IN CHAPTER 1, GENERAL INFORMATION, OF ALL FUTURE VOLUMES OF THE UNITED STATES COAST PILOT. THE 1994 EDITION OF UNITED STATES COAST PILOT 6, SCHEDULED FOR PUBLIC RELEASE IN MARCH 1994, WILL BE THE FIRST VOLUME TO INCLUDE THE EXPLANATORY TEXT AND MATRIX. BASED UPON THESE ACTIONS, RECOMMENDATION M-93-28 HAS BEEN CLASSIFIED "CLOSED--ACCEPTABLE ACTION."

THIS DIRECTIVE SPECIFIES THE PLACEMENT OF SOURCE DIAGRAMS ON ALL CHART 1:500,000 SCALE OR LARGER. IN ADDITION, AN EXPLANATORY TEXT AND MATRIX REGARDING THE ERAS OF HYDROGRAPHIC SURVEY TECHNOLOGY HAVE BEEN FINALIZED. THIS TEXT AND MATRIX WILL BE PUBLISHED IN CHAPTER 1, GENERAL INFORMATION, IN ALL VOLUMES OF THE UNITED STATES COAST PILOT AS THEY ARE PREPARED FOR PRINTING. THE FIRST VOLUME TO BE AFFECTED BY THIS INCLUSION OF INFORMATION IS THE 1994 EDITION OF THE UNITED STATES COAST PILOT 6, SCHEDULED FOR PUBLIC RELEASE IN MARCH 1994. NOAA REQUESTS THAT THE STATUS OF RECOMMENDATION M-93-28 CLASSIFIED AS "CLOSED-ACCEPTABLE ACTION."

Response: THE BOARD IS PLEASED THAT NOAA IS PLANNING A REVISION TO CHAPTER 1 OF THE UNITED STATES COAST PILOT THAT EXPLAINS HYDROGRAPHIC SURVEY TECHNOLOGY AND REQUIRE A HYDROGRAPHIC SURVEY COVERAGE DIAGRAM BE INCLUDED ON ALL NAUTICAL CHARTS 1:500,000 SCALE OR LARGER. THE BOARD WOULD APPRECIATE A COPY OF THE CARTOGRAPHIC ORDER EFFECTING THESE CHANGES WHEN IT IS PUBLISHED. IN THE ITERIM, RECOMMENDATION M-93-28 HAS BEEN CLASSIFIED "OPEN--ACCEPTABLE RESPONSE."

Response: IN THE FALL OF 1992, NOAA'S NOS DEVELOPED A CARTOGRAPHIC POLICY CONCERNING SURVEY INFORMATION TO BE INCLUDED ON NOS NAUTICAL CHARTS AND IN THE UNITED STATES COAST PILOT. A DRAFT REVISION TO THE NAUTICAL CHART MANUAL, VOLUME 1, SEVENTH (1992) EDITION INCORPORATING THIS POLICY WAS PREPARED. THE REVISION SPECIFIES THE PLACEMENT OF HYDROGRAPHIC SURVEY COVERAGE DIAGRAMS ON ALL NOS NAUTICAL CHARTS AT THE 1:500,000 SCALE AND LARGER. THESE NEW INSTALLATIONS WILL BE IN MATRIX FORM BY AUGUST 1993. IN ADDITION, AN EXPLANATORY TEXT AND MATRIX REGARDING THE ERAS OF HYDROGRAPHIC SURVEY TEHNOLOGY ARE BEING DRAFTED FOR INCLUSION IN CHAPTER 1, GENERAL INFORMATION, UNITED STATES COAST PILOT. NOAA BELIEVES THAT PUBLICATION OF THE DIAGRAMS AND RELATED CHANGES TO THE COAST PILOT WILL SERVE AS A GUIDE TO NAVIGATORS OF DEEP-DRAFT VESSELS.
