Synopsis: ON AUGUST 7, 1992, THE UNITED KINGDOM PASSENGER VESSEL RMS (ROYAL MAIL SHIP) QUEEN ELIZABETH 2 (QE2) WAS OUTBOUND IN VINEYARD SOUND, MASSACHUSETTS, WHEN THE VESSEL GROUNDED ABOUT 2 1/2 MILES SOUTH OF CUTTYHUNK ISLAND. NO INJURIES OR DEATHS RESULTED FROM THIS ACCIDENT. HOWEVER, DAMAGE WAS SIGNIFICANT; TEMPORARY AND PERMANENT REPAIRS COST ABOUT $13.2 MILLION. IN ADDITION, THE TOTAL REVENUE LOST FOR THE PERIOD BEFORE THE VESSEL RETURNED TO SERVICE ON OCTOBER 2, 1992, WAS ESTIMATED AT $50 MILLION.
Recommendation: THE NTSB RECOMMENDS THAT THE NATIONAL OCEANIC AND ATMOSPHERIC ADMINISTRATION: INCLUDE A DESCRIPTION OF THE WESTERN APPROACH TO VINEYARD SOUND FOR DEEP-DRAFT VESSELS IN VOLUME 2, CHAPTER 5, OF THE U.S. COAST PILOT.

Response: THE BOARD NOTES THAT NOAA DISSEMINATED THE RECOMMENDED INFORMATION BY PUBLISHING A NOTICE TO MARINERS AMENDING THE 1993 EDITION OF UNITED STATES COAST PILOT 2 AND THAT NOAA WILL INCLUDE THIS INORMATION IN THE 1994 EDITION. THEREFORE, RECOMMENDATION M-93-29 HAS BEEN CLASSIFIED "CLOSED--ACCPETABLE ACTION."

TEXT HAS BEEN DRAFTED TO REVISE THE CURRENT DESCRIPTION OF THIS APPROACH TO "DEEP-DRAFT VESSELS ENTERING VINEYARD SOUND FROM THE WEST SHOULD STAY AT LEAST 3.5 MILES SOUTHWARD OF THE SOUTHWEST END OF CUTTYHUNK ISLAND THEN PROCEED TO PASS SOUTHEAST OF THE NA BUOY." THE 1993 EDITION OF UNITED STATES COAST PILOT 2 IS PRESENTLY BEING PRINTED AND IS SCHEDULED FOR PUBLIC RELEASE IN AUGUST 1993. THE REVISED TEXT WILL BE BROADCAST AS A NOTICE TO MARINERS (NM) RELEASE TO UPDATE THE 1993 EDITION AND WILL BE PUBLISHED IN THE 1994 EDITION. NOAA REQUESTS THAT RECOMMENDATION M-93-29 BE CLASSIFIED "OPEN--ACCEPTABLE ACTION" PENDING NM PUBLICATION OF A REVISED COAST PILOT TEXT FOR THE APPROACHES TO VINEYARD SOUND.
