Synopsis: ABOUT 8:35 P.M. ON 12/3/94, AN ENGINERMAN FIRE OCCURRED ABOARD THE U.S. SMALL PASSENGER VESSEL ARGO COMMODORE WHILE ON A DINNER CRUISE IN SAN FRANCISCO BAY. ALL PASSENGER WERE SAFELY EVACUATED BY A USCG VESSEL & A PASSING YACHT; THERE WERE NO DEATHS OR INJURIES AMONG THE 41 PASSENGERS & 4 CREW MEMBERS.
Recommendation: THE NTSB RECOMMENDS THAT THE COMMODORE DINING CRUISES, INC.: CONDUCT A THOROUGH EVALUATION & REPAIR OF THE ELECTRICAL STARTING SYSTEM OF THE ARGO COMMORDORE & IMPLEMENT A MANAGEMENT OVERSIGHT PROGRAM TO ENSURE ADEQUATE MAINTENANCE FOR ALL VESSELS IN YOUR FLEET.

Response: M-95-45 ASKED THAT COMMODORE CONDUCT A THOROUGH EVALUATION & REPAIR OF THE ELECTRICAL STARTING SYSTEM OF THE ARGO COMMODORE & IMPLEMENT A MANAGEMENT OVERSIGHT PROGRAM TO ENSURE ADEQUATE MAINTENANCE FOR ALL VESSELS IN ITS FLEET. BECAUSE COMMODORE HAS DONE THE EVALUATION & IMPLEMENTED THE PROGRAM, M-95-45 HAS BEEN CLASSIFIED "CLOSED--ACCEPTABLE ACTION."

Response: CAPTAINS HAVE MAINTENANCE SHEETS IN THEIR LOG BOOKS ON WHICH THEY LIST ANY MAINTENANCE NEEDING TO BE PERFORMED, I.E., ON THE BRIDGE, STARBOARD GENSET DEVELOPED A FUEL LEAK AT THE FILTER; SWITCHED TO PORT GENSET; REPAIR BEFORE NEXT OPERATION, SLIGHT OIL LEAK DETECTED ON THE PORT MAIN, OUTBOARD; TORQUE OR REPLACE GASKET, ETC. WE HAVE MAINTENANCE RECORDS FOR EACH VESSEL & ALSO RECORD MAINTENANCE PERFORMED IN THE VESSEL'S LOG. IN 1995 WE PAID 55,000 TO BOB ROOT OF ISLAND MARINE WHO HANDLES THE HEAVY MAINTENANCE FOR THE FLEET, I.E. TRANSMISSION OVERHAUL. OUR CURRENT POLICY IS TO REPLACE OLD ENGULFS WITH NEW ONES, RATHER THAN TO OVERHAUL. THIS WAY WE GET LATEST TECHNOLOGY & GENERALLY A SMALLER, MORE COMPACT ENGINE WITH THE SAME OR GREATER POWER. THIS IS OF PARTICULAR IMPORTANCE TO US REGARDING DIE GENSET, THE VERY HEART OF A DINING VESSEL. WE PURCHASED TWO NEW MAINS, THIS YEAR, & ONE NEW GENSET.
