Synopsis: ON 8/21/94, A DISABLED 18-FOOT QUESTAR MOTOR BOAT WITH THE VESSEL'S OWNER & ONE PASSENGER ABOARD CAPSIZED WHILE BEING TOWED BY THE COAST GUARD AUXILIARY VESSEL PUPPET IMMEDIATELY SOUTH OF SHELTER ISLAND, LYNN CANAL, NEAR JUNEAU, ALASKA. THE QUESTAR'S OWNER, WHO WAS MILDLY DISABLED, WAS TRAPPED INSIDE THE VESSEL'S CABIN & WAS DROWNED. THE PASSENGER WAS NOT INJURED.
Recommendation: THE NTSB RECOMMENDS THAT THE USCG: PROVIDE RISK ASSESSMENT TRAINING TO ALL COAST GUARD AUXILIARY PERSONNEL INVOLVED IN SEARCH & RESCUE MISSIONS.

Response: THE BOARD IS PLEASED THAT RISK ASSESSMENT AS PART OF TEAM COORDINATION TRAINING (TCT) IS NOW A PART OF AN AUXILIARIT'S TRAINING. FURTHER, AN EXPLANATION FOR THE NEED FOR TCT WILL BE INCLUDED IN THE REVISION OF THE USCG BOAT CREW SEAMANSHIP MANUAL; CONSEQUENTLY, RISK ASSESSMENT TASKS WILL BE ADDED TO THE AUXILIARY'S BOAT CREW TRAINING & QUALIFICATION PROGRAM. BECAUSE THESE ACTIONS SATISFY THE INTENT OF THE RECOMMENDATION, M-96-14 HAS BEEN CLASSIFIED "CLOSED--ACCEPTABLE ACTION."

Response: WE CONCUR WITH THIS RECOMMENDATION. RISK ASSESSMENT HAS ALREADY BECOME PART OF AUXILIARY TRAINING AS PART OT TEAM COORDINATION TRAINING (TCT). THE USCG ALREADY HAS TRAINED AUXILIARIST AS TCT FACILITATORS, & ADDITIONAL AUXILIARISTS ARE SCHEDULED TO BE TRAINED BY THIS SUMMER. TCT WILL ALSO BE INCLUDED IN THE REVISION OF THE USCG BOAT CREW SEAMANSHIP MANUAL, WITH RISK ASSESSMENT TASKS ADDED TO THE AUXILIARY'S BOAT CREW TRAINING & QUALIFICATION PROGRAM AS A RESULT. WE REQUEST THAT THE STATUS OF THIS RECOMMENDATION BE CHANGED TO 'CLOSED ACCEPTABLE ACTION."
