Details
Synopsis: ON 8/21/94, A DISABLED 18-FOOT QUESTAR MOTOR BOAT WITH THE VESSEL'S OWNER & ONE PASSENGER ABOARD CAPSIZED WHILE BEING TOWED BY THE COAST GUARD AUXILIARY VESSEL PUPPET IMMEDIATELY SOUTH OF SHELTER ISLAND, LYNN CANAL, NEAR JUNEAU, ALASKA. THE QUESTAR'S OWNER, WHO WAS MILDLY DISABLED, WAS TRAPPED INSIDE THE VESSEL'S CABIN & WAS DROWNED. THE PASSENGER WAS NOT INJURED.
Recommendation: THE NTSB RECOMMENDS THAT THE USCG: INCORPORATE THE LESSONS LEARNED FROM THIS ACCIDENT INTO A CASE STUDY TRAINING EXERCISE FOR USCG SEARCH & RESCUE UNITS, INCLUDING USCG RESERVE & AUXILIARY COMMANDS.

Response: Safety Board staff, in discussion with senior search and rescue program staff at Coast Guard headquarters, verified that the lessons learned from this accident have been incorporated into a case study training exercise for Coast Guard search and rescue units, including Coast Guard Reserve and Auxiliary commands. In addition, the Safety Board understands that Team Coordination Training has been added to all Auxiliary Operations Workshops. Accordingly, Safety Recommendation M-96-17 is classified "Closed--Acceptable Action."
