Safety Recommendation History
From: NTSB
To: Norwegian Cruise Line, Orient Lines
Date: 3/15/1999
Response: THE BOARD IS PLEASED TO LEARN THAT ORIENT LINES, INC. HAS HAD A GENERAL DUCT-CLEANING PROGRAM IN PLACE FOR ITS VESSEL UTILIZING ULTRA-SOUND, VACUUM, AND SANITIZING EQUIPMENT. IN ADDITION, THE BOARD IS PLEASED THAT ORIENT LINES, INC. HAS INSTIGATED A PRIORITY CONCENTRATION ON INSPECTING AND CLEANING LAUNDRY VENTILATION SYSTEMS AND THAT PROGRAM WILL CONTINUE TO BE PERFORMED ON AN ON-GOING BASIS. BECAUSE THE INSPECTIONS AND CLEANINGS WILL CONTINUE AS REQUESTED, M-98-125 AND -126 HAVE BEEN CLASSIFIED "CLOSED--ACCEPTABLE ACTION."

From: Norwegian Cruise Line, Orient Lines
To: NTSB
Date: 11/18/1998
Response: Letter Mail Controlled 11/24/98 1:34:21 PM MC# 981396 WE WOULD LIKE TO THANK YOU FOR SENDING US A COPY OF THE NTSB RECOMMENDATIONS REFERENCED ABOVE. WE WOULD LIKE TO INFORM YOU THAT ORIENT LINES ALREADY HAD A GENERAL DUCT-CLEANING PROGRAM IN PLACE FOR OUR VESSEL UTILIZING ULTRA-SOUND, VACUUM, AND SANITIZING EQUIPMENT. HOWEVER, IN LIGHT OF NTSB'S RECOMMENDATIONS, WE HAVE INSTIGATED A PRIORITY CONCENTRATION ON INSPECTING AND CLEANING OF LAUNDRY VENTILATION SYSTEMS. SUCH INSPECTION AND CLEANING WILL CONTINUE TO BE PERFORMED ON AN ON-GOING BASIS IN ACCORDANCE WITH M-98-125 AND -126.

From: NTSB
To: Commodore Cruise Line
Date: 2/19/1999
Response: THE SAFETY BOARD IS PLEASED TO LEARN THAT THE LAUNDRY VENTILATION SYSTEMS OF COMMODORE CRUISE LINES' VESSELS ENCHANTED ISLE AND UNIVERSE EXPLORER WERE INSPECTED SUBSEQUENT TO THE FIRE ON BOARD CARNIVAL'S ECSTASY AND THAT EACH VESSEL HAS RECEIVED PERIODIC INSPECTIONS AND MAINTENANCE OF ITS MAIN LAUNDRY VENTILATION SYSTEMS SINCE THE VESSELS WERE PLACED UNDER V. SHIPS MARINE, LTD.'S MANAGEMENT. THE BOARD FURTHER UNDERSTANDS THAT INSPECTION AND MAINTENANCE HAVE BEEN INCORPORATED INTO THE VESSELS' PLANNED MAINTENANCE SYSTEM AND THAT V. SHIPS MARINE, LTD. FORWARDED COPIES OF OUR 10/20/98, LETTER TO BOTH VESSELS TO REINFORCE THE CRITICAL NATURE OF THIS MAINTENANCE IN REGARD TO FIRE SAFETY. ACCORDINGLY, M-98-125 AND -126 HAVE BEEN CLASSIFIED "CLOSED--ACCEPTABLE ACTION."

From: Commodore Cruise Line
To: NTSB
Date: 12/22/1998
Response: Letter Mail Controlled 12/29/98 1:41:22 PM MC# 981509 V. SHIPS MARINE, LTD. (VSML) ARE MANAGERS OF THE PASSENGER VESSELS, ENCHANTED ISLE AND UNIVERSE EXPLORER, ON BEHALF OF COMMODORE CRUISE LINE. COMMODORE CRUISE LINE'S PRESIDENT, MR. JAMES SULLIVAN, FORWARDED A COPY OF YOUR 10/20/98 LETTER, CONTAINING THE ABOVE REFERENCED RECOMMENDATIONS, TO OUR ATTENTION FOR REVIEW AND RESPONSE. COMMODORE'S VESSELS HAVE RECEIVED PERIODIC INSPECTIONS AND MAINTENANCE OF THEIR MAIN LAUNDRY VENTILATION SYSTEMS SINCE THEY WERE PLACED UNDER OUR MANAGEMENT. THIS INSPECTION AND MAINTENANCE WAS SUBSEQUENTLY INCORPORATED INTO THE VESSELS' PLANNED MAINTENANCE SYSTEM. WE FORWARDED COPIES OF YOUR LETTER TO BOTH VESSELS TO REINFORCE THE CRITICAL NATURE OF THIS MAINTENANCE IN REGARDS TO FIRE SAFETY. THE LAUNDRY VENTILATION SYSTEMS ON BOTH VESSELS WERE INSPECTED SUBSEQUENT TO THE FIRE ON BOARD CARNIVAL'S ECSTASY. IN VIEW OF THE FOREGOING, WE BELIEVE BOTH VESSELS ARE IN FULL COMPLIANCE WITH NTSB RECOMMENDATIONS M-98-125 AND -126. PLEASE FEEL FREE TO CONTACT US IF YOU REQUIRE ANY FURTHER INFORMATION.

From: NTSB
To: Princess Cruise Lines
Date: 3/12/1999
Response: THE SAFETY BOARD STAFF CONTACTED CAPTAIN DAVID BROWN OF YOUR STAFF TO CLARIFY THE ACTION YOU HAVE TAKEN ON THESE RECOMMENDATIONS AND FOR FURTHER INFORMATION ON THE CLEANING AND MAINTENANCE PROGRAM AT PRINCESS. MY STAFF LEARNED THAT PRINCESS HAS HAD A MAINTENANCE MANAGEMENT SYSTEM (MMS) IN PLACE FOR QUITE SOME TIME, AND THAT THE THOROUGH INSPECTION OF THE LAUNDRY ROOM VENT SYSTEM IS PART OF THE MMS PROGRAM. WE NOW UNDERSTAND THAT ALL NINE VESSELS HAVE BEEN INSPECTED IN ACCORDANCE WITH THE COMPANY MMS PROGRAM, WHICH INCORPORATED A THOROUGH INSPECTION OF THE LAUNDRY VENTILATION SYSTEMS, INCLUDING DUCTS, PLENUMS AND EXHAUST TERMINUSES. WE FURTHER UNDERSTAND THAT PRINCESS HAS CLEANED THE DUCTS ON ALL OF ITS VESSELS, AS NEEDED, TO REDUCE THE RISK OF FIRE. IN ADDITION, OUR INFORMATION INDICATES THAT THE MMS PROGRAM INCLUDES A DAILY CHECKS BY SHIPBOARD PERSONNEL OF THE FILTERS AND VENT SYSTEM IN THE VESSEL LAUNDRY ROOMS TO ENSURE THAT THE SYSTEM IS CLEAN AND CLEAR OF ANY COMBUSTIBLE MATERIAL THAT COULD POSE A FIRE HAZARD. BECAUSE PRINCESS HAS AN ONGOING LAUNDRY VENTILATION SYSTEM INSPECTION AND CLEANING PROGRAM, M-98-125 AND -126 HAVE BEEN CLASSIFIED "CLOSED--ACCEPTABLE ACTION."

From: Princess Cruise Lines
