Details
Synopsis: ON 7/20/98, A FIRE OCCURRED ON BOARD THE LIBERIAN-REGISTERED PASSENGER VESSEL ECSTASY, OPERATED BY CARNIVAL CRUISE LINES. THE VESSEL HAD JUST DEPART PORT OF MAIMI, FLORIDA, BOUND FOR KEY WEST, FLORIDA, WITH 2,557 PASSENGERS & 920 CREWMEMBERS ABOARD. AT 1710, THE SHIP'S FIRE ALARM SYSTEM ON THE BRIDGE SOUNDED, INDICATING THAT A MANUAL ALARM HAD BEEN ACTIVATED IN THE LAUNDRY ROOM. FIRE SPREAD THROUGH THE LAUNDRY VENTILATION SYSTEM, & FLAMES & LARGE VOLUMES OF SMOKE WERE SEEN ISSUING FROM THE STERN OF THE VESSEL. THE VESSEL LOST PROPULSION & STEERING AS A RESULT OF THE FIRE, WHICH WAS BROUGHT UNDER CONTROL & EXTINGUISHED ABOUT 2109. THE SHIP WAS RETURNED TO ITS BERTH AT PORT OF MIAMI AT 0220 ON JULY. ALL PASSENGERS SAFELY DISEMBARKED BEFORE 0600. NO FATALITIES OCCURRED; THE INJURED INCLUDED 14 CREWMEMBERS & 6 PASSENGERS.
Recommendation: THE NTSB RECOMMENDS THAT THE (CRUISE VESSEL OWNER & OPERATOR): INSTITUTE A PROGRAM TO VERIFY ON A CONTINUING BASIS THAT THE LAUNDRY VENTILATION SYSTEMS, INCLUDING DUCTS & PLENUMS, REMAIN CLEAN & CLEAR OF ANY COMBUSTIBLE MATERIAL THAT POSES A FIRE HAZARD ON YOUR VESSELS.

THE BOARD HAS BEEN FURTHER INFORMED THAT ROYAL OLYMPIC CRUISES OPENS "INSPECTION DOORS" AT INTERVALS ON THE LAUNDRY VENTILATION DUCTS IN ORDER TO FACILITATE PROPER CLEANING OF THE DUCTS FROM THE COLLECTED LINT AND AT THE SAME TIME TO MAKE CERTAIN THAT THE ENTIRE LENGTH OF THE DUCT IS CLEAR. ACCORDINGLY, M-98-126 HAS BEEN CLASSIFIED "CLOSED-- ACCEPTABLE ACTION."

WE NOW UNDERSTAND THAT ALL NINE VESSELS HAVE BEEN INSPECTED IN ACCORDANCE WITH THE COMPANY MMS PROGRAM, WHICH INCORPORATED A THOROUGH INSPECTION OF THE LAUNDRY VENTILATION SYSTEMS, INCLUDING DUCTS, PLENUMS AND EXHAUST TERMINUSES. WE FURTHER UNDERSTAND THAT PRINCESS HAS CLEANED TO DUCTS ON ALL OF ITS VESSELS, AS NEEDED, TO REDUCE THE RISK OF FIRE. IN ADDITION, OUR INFORMATION INDICATES THAT THE MMS PROGRAM INCLUDES A DAILY CHECKS BY SHIPBOARD PERSONNEL OF THE FILTERS AND VENT SYSTEM IN THE VESSEL LAUNDRY ROOMS TO ENSURE THAT THE SYSTEM IS CLEAN AND CLEAR OF ANY COMBUSTIBLE MATERIAL THAT COULD POSE A FIRE HAZARD. Response: The Safety Board understands that Royal Caribbean International & Celebrity Cruises, Inc., have added such inspections to their regular maintenance programs. The Safety Board understands that the inspections and cleanings are done on a monthly basis, that additional access points have been installed on some ships to facilitate the inspections, that ventilation systems from the galley areas are also inspected and cleane on a monthly basis to insure that there is no build up of grease and that additional fire suppression systems have been added in some ships. Accordingly, M-98-126 has been classified "Closed--Acceptable Action."
