Synopsis: ON FEBRUARY 21, 1980, AN EXPLOSION AND FIRE DESTROYED FOUR STORES IN A SHOPPING COMPLEX AND SEVERELY DAMAGED AN ADJOINING RESTAURANT IN CORDELE, GEORGIA. OF THE EIGHT PERSONS WHO WERE INJURED, THREE DIED LATER AS A RESULT OF THEIR INJURIES. PROPERTY DAMAGE WAS EXTENSIVE.
Recommendation: THE NTSB RECOMMENDS THAT THE RESEARCH AND SPECIAL PROGRAMS ADMINISTRATION OF THE DEPARTMENT OF TRANSPORTATION: INSPECT THE CITY OF CORDELE GAS SYSTEM FOR COMPLIANCE WITH APPLICABLE FEDERAL REGULATIONS AND TAKE NECESSARY ACTION.

Response: RESEARCH AND SPECIAL PROGRAMS ADMINISTRATION LTR: THE CITY OF CORDELLE GAS SYSTEM HAS BEEN INSPECTED BY MTB IN 1972, 1975, AND 1978. IN 1972 DEFICIENCIES IN VALVE MAINTENANCE AND RECORDS, AND WELDER QUALIFICATION WERE NOTED. THE CITY RESPONDED BY NOTING CORRECTIVE STEPS WERE BEING TAKEN. THE 1975 INSPECTION SHOWED DEFICIENCIES IN LEAKAGE SURVEYS, RECORDS, AND CORROSION CONTROL PROGRAM. THE CITY RESPONDED BY SAYING THAT STEPS WERE BEING TAKEN FOR CORRECTION. BECAUSE THE REGULATIONS WERE NEW AND THE CITY WAS WORKING TOWARD CORRECTION OF DEFICIENCIES, MTB TOOK NO FURTHER ACTION. IN 1978 DEFICIENCIES WERE FOUND IN MAINTENANCE PLANS, ODORIZATION, VALVE MAINTENANCE, ABANDONMENT OR INACTIVATION PROCEDURES, TESTING OF RELIEF VALVES, AND EXTERNAL CORROSION CONTROL. THE CITY WAS ASSESSED AND PAID A $1,000 CIVIL PENALTY. ON FEBRUARY 21-23, 1980, AFTER THE FEBRUARY 21 ACCIDENT, MTB INSPECTED THE CORDELE SYSTEM FINDING FOUR POSSIBLE VIOLATIONS. A PRELIMINARY CIVIL PENALTY OF $6,200 WAS ASSESSED. THE VIOLATIONS WERE: (1) FAILURE TO MAKE A TIMELY REPORT OF THE FEBRUARY 21, 1980 ACCIDENT. (2) LACK OF A CONTINUING PUBLIC EDUCATION PLAN. (3) LACK OF ADEQUATE ODORIZATION PROGRAM. (4) FAILURE TO PROMPTLY REPAIR, REPLACE, OR REMOVE FROM SERVICE A PIPELINE KNOWN TO BE UNSAFE, THE CITY REQUESTED A HEARING AND ON JULY 10, 1980, ONE WAS CONDUCTED. THE CITY OFFERED A COMPROMISE PAYMENT OF $4,800 WHICH WAS ACCEPTED AND THE CITY WAS DIRECTED TO CORRECT THE VIOLATIONS. THE NEXT SCHEDULED INSPECTION OF THE SYSTEM IS 1983. MTB WILL MONITOR THE CITY'S PROGRESS REPORTS UNTIL THEN.
