Synopsis: ON NOVEMBER 7, 1980, CONRAIL FREIGHT TRAIN OPSE-7 STRUCK THE HEAD END OF AMTRAK TRAIN NO. 74 WHILE IT WAS STANDING ON TRACK NO. 2 AT DOBBS FERRY, NEW YORK. THE LEAD LOCOMOTIVE UNIT OF TRAIN OPSE-7 OVERRODE AND DESTROYED THE OPERATING CAB OF THE POWER CAR OF TRAIN NO. 74. OF THE ESTIMATED 234 PERSONS ABOARD THE TRAINS, 75 PASSENGERS AND 9 CREWMEMBERS WERE INJURED. DAMAGE TO THE EQUIPMENT WAS ESTIMATED AT $915,000.
Recommendation: THE NTSB RECOMMENDS THAT THE FEDERAL RAILROAD ADMINISTRATON: CONDUCT A SAFETY REVIEW OF THE METROPOLITAN REGION TO DETERMINE WHY THE ACTUAL OPERATION OF TRAINS WAS NOT IN COMPLIANCE WITH CONRAIL RULES, AND PROVIDE THE SAFETY BOARD A REPORT OF THE FINDINGS.

Response: FRA LETTER: FRA INITIATED A SPECIAL ASSESSMENT PROGRAM ON THE METROPOLITAN REGION OF CONRAIL IN OCTOBER 1980. THE INVESTIGATION REVEALED 68 INCIDENTS THAT INVOLVED 150 OPERATING RULES VIOLATIONS FROM OCTOBER 1980 TO MARCH 1981. CONRAIL MANAGEMENT HAS RESTRUCTURED THE METROPOLITAN REGION SAFETY DEPARTMENT AND EMPHASIZED THE NEED FOR ADEQUATE EMPLOYEE TRAINING AND TESTING. EACH EMPLOYEE WILL BE OBSERVED TWICE ANNUALLY WHILE PERFORMING HIS OR HER DUTIES, AND WILL RECEIVE PROPER INSTRUCTION CONCERNING ANY SAFETY RULE VIOLATIONS. THE OBSERVATIONS WILL BE DOCUMENTED THROUGH CONRAIL'S OPERATIONAL AND SAFETY TESTS SYSTEMS.

THE INVESTIGATION REVEALED 68 INCIDENTS THAT INVOLVED 150 OPERATING RULES VIOLATIONS FROM OCTOBER 1980 TO MARCH 1981. IT ALSO INDICATED THAT THE ACCIDENTS OCCURRING IN THE REGION WERE ATTRIBUTED TO HUMAN ERROR AND APPEARED TO BE THE RESULT OF INADEQUATE EMPLOYEE TRAINING AND TESTING IN OPERATING RULES. CONRAIL MAMAGEMENT HAS RESTRUCTURED THE METROPOLITAN REGION SAFETY DEPARTMENT AND EMPHASIZED THE NEED FOR ADEQUATE EMPLOYEE TRAINING AND TESTING. EACH EMPLOYEE WILL BE OBSERVED TWICE ANNUALLY WHILE PERFORMING THEIR DUTIES. THE EMPLOYEE WILL RECEIVE PROPER INSTRUCTION CONCERNING ANY SAFETY RULE VIOLATION. THE OBSERVATIONS WILL BE DOCUMENTED THROUGH CONRAIL'S OPERATIONAL AND SAFETY TESTS SYSTEMS. FRA WILL CONTINUE TO MONITOR THIS PROGRAM AND OBTAIN FULL COMPLIANCE WITH FEDERAL REGULATIONS AND REQUESTS THAT SAFETY RECOMMENDATION R-81-52 BE CLOSED.
