Synopsis: ABOUT 8:12 A.M., EASTERN DAYLIGHT TIME, JULY 10, 1985, EAST BOUND TWO-CAR TRAIN NO. 6601 STRUCK THE REAR OF THREE-CAR TRAIN NO. 6614, WHICH WAS STANDING INOPERATIVE ON THE EAST BOUND MAIN TRACK OF THE GREATER CLEVELAND REGIONAL TRANSIT AUTHORITY (GCRTA) RED RAPID TRANSIT LINE. TRAIN NO. 6614 WAS STUCK IN A REVERSE CURVE ABOUT 900 FEET WEST OF THE WEST 98TH STREET STATION IN CLEVELAND, OHIO. THE OPERATORS AND CONDUCTORS OF BOTH TRAINS AND A TOTAL OF 46 OF THE APPROX IMATELY 400 PASSENGERS ON THE TRAINS WERE TRANSPORTED TO NEARBY HOSPITALS. TWO DAYS AFTER THE ACCIDENT, ANOTHER PASSENGER WAS ADMITTED TO THE HOSPITAL FOR A CERVICAL SPINE INJURY; THE TRAIN CREWMEMBERS AND THE OTHER PASSENGERS RE CEIVED OUTPATIENT TREATMENT FOR MINOR INJURIES. THE REAR CAR OF TRAIN NO. 6614 WAS DERAILED AND SUSTAINED REAR END STRUCTURAL DAMAGE AS A RESULT OF THE COLLISION IMPACT.
Recommendation: THE NTSB RECOMMENDS THAT THE GREATER CLEVELAND REGIONAL TRANSIT AUTHORITY: MODIFY THE RAIL OPERATIONS RULE BOOK TO SPECIFY, FOR CONDITIONS REQUIRING THE OPERATION OF A TRAIN PAST A STOP SIGNAL, A MAXIMUM RESTRICTED SPEED THAT WOULD ENABLE THE TRAIN TO BE STOPPED SHORT OF A STANDING TRAIN, A BROKEN RAIL, OR ANY OTHER HAZARD.

Response: IN THOSE CASES WHEN A TRAIN IS STANDING AT A RED OR DARK SIGNAL AND IT HAS BEEN DETERMINED THAT THE BLOCK AHEAD IS NOT OCCUPIED, THE TOWER CONTROL SUPERVISOR MAY ALLOW THE TRAIN TO PROCEED UNDER A RESTRICTIVE MOVEMENT PROCEDURE. THE OPERATOR'S RULE BOOK HAS BEEN AMENDED WITH SPECIFIC LANGUAGE REFERRING TO RED SIGNAL ASPECTS, CITING A MAXIMUM RESTRICTED SPEED OF 15 MPH. THE RULE BOOK READS AS FOLLOWS: UPON BEING GIVEN PERMISSION TO ENTER A RED BLOCK, OPERATOR WILL TRAVEL AT A RESTRICTED SPEED NOT TO EXCEED 15 MPH UP TO THE NEXT SIGNAL. WHILE OPERATING AT THIS RESTRICTED SPEED (15 MPH), BE PREPARED TO STOP WITHIN TEN (10) FEET OF A STANDING TRAIN, BROKEN RAIL, OR HAZARDOUS FOREIGN OBJECTS OR CONDITION. PERSONNEL WILL THEN NOTIFY THE TOWER CONTROL SUPERVISOR OF ANY AND ALL CONDITIONS DISCOVERED, EVEN IF CLEAR.
