Synopsis: ABOUT 8:12 A.M., EASTERN DAYLIGHT TIME, JULY 10, 1985, EAST BOUND TWO-CAR TRAIN NO. 6601 STRUCK THE REAR OF THREE-CAR TRAIN NO. 6614, WHICH WAS STANDING INOPERATIVE ON THE EAST BOUND MAIN TRACK OF THE GREATER CLEVELAND REGIONAL TRANSIT AUTHORITY (GCRTA) RED RAPID TRANSIT LINE. TRAIN NO. 6614 WAS STUCK IN A REVERSE CURVE ABOUT 900 FEET WEST OF THE WEST 98TH STREET STATION IN CLEVELAND, OHIO. THE OPERATORS AND CONDUCTORS OF BOTH TRAINS AND A TOTAL OF 46 OF THE APPROX IMATELY 400 PASSENGERS ON THE TRAINS WERE TRANSPORTED TO NEARBY HOSPITALS. TWO DAYS AFTER THE ACCIDENT, ANOTHER PASSENGER WAS ADMITTED TO THE HOSPITAL FOR A CERVICAL SPINE INJURY; THE TRAIN CREWMEMBERS AND THE OTHER PASSENGERS RE CEIVED OUTPATIENT TREATMENT FOR MINOR INJURIES. THE REAR CAR OF TRAIN NO. 6614 WAS DERAILED AND SUSTAINED REAR END STRUCTURAL DAMAGE AS A RESULT OF THE COLLISION IMPACT.
Recommendation: THE NTSB RECOMMENDS THAT THE GREATER CLEVELAND REGIONAL TRANSIT AUTHORITY: MODIFY THE RADIO RULES TO REQUIRE, WHEN HAZARDS SUCH AS A DISABLED TRAIN EXIST, TOWER CONTROL SUPERVISORS TO NOTIFY THE OPERATORS OF FOLLOWING TRAINS AND TRAINS ON ADJACENT TRACKS OF THE HAZARD AND PROHIBIT THEM FROM ENTERING THAT BLOCK WITHOUT SPECIFIC AUTHORITY TO DO SO.

Response: RADIO RULES HAVE BEEN MODIFIED. ALL OPERATORS HAVE BEEN INFORMED OF THE CHANGE. THE RAIL OPERATING ORDER EFFECTING THAT CHANGE IS ATTACHED. ALL OPERATING AND SUPERVISORY PERSONNEL ARE WELL AWARE OF ANY CHANGES IN PROCEDURES AFFECTING THE MOVEMENT OF TRAINS.

Response: COMPLIANCE WITH THIS RECOMMENDATION WAS EFFECTED IMMEDIATELY FOLLOWING THE JULY 10, 1985, ACCIDENT BY MEANS OF VERBAL INSTRUCTION TO THE TOWER CONTROL SUPERVISORS. THIS HAS BEEN REINFORCED BY MEMORANDUM, ADDRESSED TO THE TOWER CONTROL SUPERVISORS, WHICH READS IN PART: "WHENEVER A OPERATOR/ SUPERVISOR NOTIFIES ANY TOWER CONTROL SUPERVISOR THAT HIS/HER TRAIN HAS BECOME DISABLED OR DEFECTIVE AND IS UNABLE TO MOVE, THE FOLLOWER ALONG WITH THE TRAIN APPROACHING THE DISABLED OR DEFECTIVE CAR(S) ON THE OPPOSITE TRACK WILL BE NOTIFIED IMMEDIATELY... IF THE STANDING PROBLEM CANNOT BE RESOLVED... A GENERAL ANNOUNCEMENT TO ALL TRAINS THAT WILL BE AFFECTED WILL BE MADE."
