Synopsis: ABOUT 8:12 A.M., EASTERN DAYLIGHT TIME, JULY 10, 1985, EAST BOUND TWO-CAR TRAIN NO. 6601 STRUCK THE REAR OF THREE-CAR TRAIN NO. 6614, WHICH WAS STANDING INOPERATIVE ON THE EAST BOUND MAIN TRACK OF THE GREATER CLEVELAND REGIONAL TRANSIT AUTHORITY (GCRTA) RED RAPID TRANSIT LINE. TRAIN NO. 6614 WAS STUCK IN A REVERSE CURVE ABOUT 900 FEET WEST OF THE WEST 98TH STREET STATION IN CLEVELAND, OHIO. THE OPERATORS AND CONDUCTORS OF BOTH TRAINS AND A TOTAL OF 46 OF THE APPROX IMATELY 400 PASSENGERS ON THE TRAINS WERE TRANSPORTED TO NEARBY HOSPITALS. TWO DAYS AFTER THE ACCIDENT, ANOTHER PASSENGER WAS ADMITTED TO THE HOSPITAL FOR A CERVICAL SPINE INJURY; THE TRAIN CREWMEMBERS AND THE OTHER PASSENGERS RE CEIVED OUTPATIENT TREATMENT FOR MINOR INJURIES. THE REAR CAR OF TRAIN NO. 6614 WAS DERAILED AND SUSTAINED REAR END STRUCTURAL DAMAGE AS A RESULT OF THE COLLISION IMPACT.
Recommendation: THE NTSB RECOMMENDS THAT THE GREATER CLEVELAND REGIONAL TRANSIT AUTHORITY: PERIODICALLY TRAIN AND EXAMINE ALL RAIL TRAIN SERVICE EMPLOYEES AND RAIL SUPERVISORS ON THE OPERATING RULES, OPERATING PROCEDURES, AND BULLETIN INSTRUCTIONS.

Response: AT THIS DATE, ALL OPERATORS AND SUPERVISORS HAVE BEEN RETRAINED. THE ANNUAL SAFETY AND REFRESHER TRAINING PROGRAM WAS PRESENTED BETWEEN SEPTEMBER 15, 1987 AND NOVEMBER 20, 1987. 123 OPERATORS AND 29 SUPERVISORS PARTICIPATED. ALL TRAINING IS DOCUMENTED AND FILED. ATTACHED ARE SOME EXAMPLES OF DOCUMENTATION OF THIS TRAINING. (EXHIBITS) THIS COURSE WILL BE REPEATED THIS YEAR, BEGINNING SEPTEMBER 19, 1988.

Response: ALL RAIL OPERATORS RECEIVE ANNUAL TRAINING ON OPERATING RULES AND PROCEDURES, UNDER AN ENHANCED PROGRAM CALLED THE ANNUAL SAFETY AND REFRESHER TRAINING PROGRAM. THIS TRAINING INCLUDES A TEST ON OPERATING RULES, SAFETY PROCEDURES, AND TRAIN TROUBLE-SHOOTING. PARTICULAR CONCERN IS GIVEN TO CONSTANTLY UPDATING THIS PROGRAM AS CONDITIONS CHANGE THAT REQUIRE THE ISSUE OF BULLETINS OR THE MODIFICATION OF OPERATING RULES. AS OPERATORS COMPLETE THIS TRAINING IT IS FULLY DOCUMENTED IN THEIR PERSONNEL RECORDS AND RAIL TRAINING DEPARTMENT RECORDS. THE SAFETY DEPARTMENT HAS REVIEWED THE TRAINING PROGRAM AND MADE SEVERAL RECOMMENDATIONS THAT HAVE BEEN ADOPTED TO HELP REINFORCE SAFETY IN THE CLASSES. THE EXAMINATION THAT IS USED IN THE TRAINING PROGRAM (EACH OPERATOR MUST RECEIVE A PASSING GRADE ON THE EXAM) WAS COMPLETELY REVISED TO MORE ACCURATELY ASSESS THEIR UNDERSTANDING OF THE OPERATING RULES AND PROCEDURES. SAFETY PERSONNEL CONTINUALLY WORK WITH THE TRAINING INSTRUCTORS TO HELP INSURE SAFETY IS REINFORCED IN ALL TRAINING PROGRAMS AS WELL AS REMEDIAL TRAINING. ON OCCASION, SAFETY PERSONNEL PROVIDE LECTURES ON SAFETY TO OPERATORS' AND SUPERVISORS' TRAINING CLASSES. THIS TRAINING PROGRAM HAS BEEN EXTENDED TO THE RAIL SUPERVISORS AND TOWER CONTROL SUPERVISORS DURING 1987, WITH EMPHASIS ON RULES AND PROCEDURES AND ASSISTANCE TO THE OPERATORS ON THE LINE. RAIL SUPERVISORS ALSO MEET QUARTERLY TO REVIEW CURRENT BULLETINS AND THE EFFECTIVENESS OF OPERATING RULES AND PROCEDURES.
