Synopsis: ABOUT 8:12 A.M., EASTERN DAYLIGHT TIME, JULY 10, 1985, EAST BOUND TWO-CAR TRAIN NO. 6601 STRUCK THE REAR OF THREE-CAR TRAIN NO. 6614, WHICH WAS STANDING INOPERATIVE ON THE EAST BOUND MAIN TRACK OF THE GREATER CLEVELAND REGIONAL TRANSIT AUTHORITY (GCRTA) RED RAPID TRANSIT LINE. TRAIN NO. 6614 WAS STUCK IN A REVERSE CURVE ABOUT 900 FEET WEST OF THE WEST 98TH STREET STATION IN CLEVELAND, OHIO. THE OPERATORS AND CONDUCTORS OF BOTH TRAINS AND A TOTAL OF 46 OF THE APPROX IMATELY 400 PASSENGERS ON THE TRAINS WERE TRANSPORTED TO NEARBY HOSPITALS. TWO DAYS AFTER THE ACCIDENT, ANOTHER PASSENGER WAS ADMITTED TO THE HOSPITAL FOR A CERVICAL SPINE INJURY; THE TRAIN CREWMEMBERS AND THE OTHER PASSENGERS RE CEIVED OUTPATIENT TREATMENT FOR MINOR INJURIES. THE REAR CAR OF TRAIN NO. 6614 WAS DERAILED AND SUSTAINED REAR END STRUCTURAL DAMAGE AS A RESULT OF THE COLLISION IMPACT.
Recommendation: THE NTSB RECOMMENDS THAT THE GREATER CLEVELAND REGIONAL TRANSIT AUTHORITY: ISSUE AND PROVIDE TO EACH TRAIN OPERATOR AND TOWER CONTROL SUPERVISOR, A RAIL SAFETY BULLETIN ADVISING OF A SIGNAL SYSTEM FAILURE OR MALFUNCTION, SUCH AS A BROKEN TRIP ARM OR INOPERATIVE SIGNAL, EACH TIME SUCH OCCURS IF THE FAILURE OR MALFUNCTION CANNOT BE REPAIRED IMMEDIATELY.

Response: A MALFUNCTION, SUCH AS A BROKEN TRIP ARM, DOES TRIGGER NOTIFICATION PROCEDURES. THE TOWER CONTROL SUPERVISORS MAKE REGULAR RADIO ANNOUNCEMENTS IMMEDIATELY AFTER BEING INFORMED, ANYTIME A SIGNAL SYSTEM FAILURE OR MALFUNCTION OCCURS. THE TOWER CONTROL SUPERVISORS ARE THE FIRST NOTIFIED, USUALLY BY THE FIRST OPERATOR OBSERVING THE FAILURE. IN ADDITION, SIGNAL REPAIR PERSONNEL ARE DISPATCHED TO THE SITE TO EITHER MAKE IMMEDIATE REPAIRS, OR TO ATTEND THE SITE UNTIL SUCH TIME THAT REPAIRS ARE COMPLETED. IF THE REPAIR CANNOT BE COMPLETED WITHIN AN 8 HOUR PERIOD, WRITTEN NOTICE IS GIVEN TO ALL OPERATING AND SUPERVISORY PERSONNEL. THIS IS IN ADDITION TO THE RADIO ANNOUNCEMENTS MENTIONED ABOVE. IF THE REPAIR REQUIRES MORE THAN 24 HOURS TO COMPLETE, THE SIGNAL IS REMOVED FROM SERVICE, THEREBY EXTENDING THE BLOCK WHICH INCREASES THE SAFETY FACTOR. THIS IS DONE IN ADDITION TO WRITTEN NOTIFICATION AND RADIO ANNOUNCEMENTS.

Response: DURING ROUTINE SIGNAL MAINTENANCE AND REPAIR WORK BEING DONE BY SIGNAL DEPARTMENT PERSONNEL, THE OPERATING RULES GOVERNING WORK AREAS ALONG THE RIGHT-OF-WAY ARE IN EFFECT. IN THE EVENT OF A SIGNAL SYSTEM FAILURE THAT CANNOT BE REPAIRED WITHIN AN 8 HOUR PERIOD, NOTICE IS GIVEN BY THE SIGNAL DEPARTMENT TO THE TOWER CONTROL SUPERVISOR FOR REGULAR ANNOUNCEMENTS OVER THE RADIO. IF THE SIGNAL CANNOT BE REPAIRED WITHIN TWENTY-FOUR HOURS, IT WILL BE REMOVED FROM SERVICE, THEREBY EXTENDING THE BLOCK. A BULLETIN TO THAT EFFECT HAS BEEN ISSUED TO ALL TOWER CONTROL SUPERVISORS, RAIL SUPERVISORS AND RAIL OPERATORS BY THE DIRECTOR OF RAIL OPERATIONS. THIS POLICY HAS BEEN INCORPORATED IN THE RAIL EMERGENCY PROCEDURES MANUAL FOR RAIL SUPERVISION. THIS PROCEDURE WAS PUT INTO EFFECT CONCURRENTLY WITH THE POLICY REQUIRING IMMEDIATE REPAIR OF TRAIN STOPS DATED SEPTEMBER 19, 1985, REFERRED TO UNDER RECOMMENDATION R-87-9.
