Synopsis: AT 4:36 A.M. ON 5/16/94, THE SOUTHBOUND NATIONAL RAILROAD PASSENGER CORPORATION (AMTRAK) TRAIN 87, SILVER METEOR, COLLIDED WITH AN INTERMODAL TRAILER THAT HAD EITHER FALLEN OR WAS FALLING FROM A FLAT CAR ON THE PASSING NORTHBOUND CSX TRANSPORTATION INC. FREIGHT TRAIN R176-15 (CSXT 176) AT SELMA, NORTH CAROLINA. AMTRAK TRAIN 87 CONSISTED OF A TWO-UNIT LOCOMOTIVE & 18 CARS; CSXT 176 CONSISTED OF A THREE-UNIT LOCOMOTIVE & 52 CARS. ALL BUT THE LAST CAR OF AMTRAK TRAIN 87 DERAILED, & THE NEXT TO THE LAST CAR ON CSXT 176 ALSO DERAILED. ON AMTRAK TRAIN 87, THE ASSISTANT ENGINEER WAS KILLED, THE ENGINEER SUSTAINED SERIOUS INJURIES, & 1 ON-BOARD SERVICE CREWMEMBER & 119 PASSENGERS RECEIVED MINOR INJURIES. THE OPERATING CREW ON CSXT 176 SUSTAINED NO INJURIES. THE NTSB DETERMINED THAT THE PROBABLE CAUSE OF THE DERAILMENT OF AMTRAK TRAIN 87 WAS THE FAILURE OF THE CSX INTERMODAL CORPORATION (CSXI) LOADING CREW TO PROPERLY SECURE THE INTERMODAL TRAILER TO THE FLAT CAR ON CSXT 176 & THE FAILURE OF CSXI TO HAVE IN PLACE A COMPREHENSIVE INSPECTION PROGRAM.
Recommendation: THE NTSB RECOMMENDS THAT THE ASSOCIATION OF AMERICAN RAILROADS: ADVISE THE NTSB WITHIN 90 DAYS OF THE PROGRESS TOWARD THE DEVELOPMENT OF THE MANUAL, POSTER, AND VIDEO FOR THE RAILROAD INDUSTRY & THE INCORPORATION OF THE RECOMMENDED PRACTICES FOR THE LOADING, SECURING, & INSPECTING OF TOFC/COFC EQUIPMENT IN MANUAL SEVEN OF THE OPEN TOP LOADING RULES. ALSO, IMPLEMENT THESE ACTIONS BY 12/31/95.

Response: R-95-22 ASKED THE ASSOCIATION OF AMERICAN RAILROADS (AAR) TO ADVISE THE BOARD WITHIN 90 DAYS OF THE PROGRESS TOWARD THE DEVELOPMENT OF THE MANUAL, POSTER, & VIDEO FOR THE RAILROAD INDUSTRY & THE INCORPORATION OF THE RECOMMENDED PRACTICES FOR LOADING, SECURING, & INSPECTING OF TRAILER-ON-FLAT-CAR/CONTAINER-ON-FLAT CAR EQUIPMENT IN MANUAL SEVEN OF THE OPEN TOP LOADING RULES. ALSO, THE RECOMMENDATION URGED THE AAR TO IMPLMENT THESE ACTIONS BY 12/31/95. WHILE IMPLEMENTATION TOOK LONGER THAN PROJECTED, THE BOARD IS PLEASED TO LEARN THAT AAR HAS ACCOMPLISHED ALL OF THE RECOMMENDED ACTIONS. BASED ON YOUR RESPONSE, THE BOARD HAS CLASSIFIED R-95-22 "CLOSED--ACCEPTABLE ACTION."

Response: ( FOLLOWUP FOR BOTH LETTERS 10/02/95 & 12/30/95) THE BOARD IS PLEASED TO HAVE RECEIVED COPIES OF NEWLY PRODUCED POSTERS & VIDEOS COVERING TRAILER HITCHES INTERMODAL CONTAINER LOADING & SECUREMENT THAT HAVE BEEN COMPLETED & DISTRIBUTED. THE BOARD ALSO NOTES THAT THE INTERMODAL TRAILER & CONTAINER SECUREMENT MANUAL WAS FIRST DISTRIBUTED IN APRIL 1995 & THAT, BECAUSE OF DEMAND, A SECOND PRINTING WAS NEEDED. THE BOARD IS ALSO AWARE THAT THE INCORPORATION OF THE RECOMMENDED PRACTICE INTO THE OPEN TOP LOADING RULES IS NEARLY COMPLETE. UNTIL THE OPEN TOP LOADING RULES ARE UPDATED, THE BOARD HAS CLASSIFIED R-95-22 "OPEN--ACCEPTABLE RESPONSE."

THE IEH TASK FORCE IS ALSO DEVELOPING A TOFC VIDEO, VARIOUS AAR RULES REVISION, & COMPREHENSIVE INDUSTRY RECOMMENDED PRACTICE COVERING INTERMODAL LOADING PROCEDURES.
