Synopsis: ABOUT 12:37 P.M. EASTERN DAYLIGHT TIME ON SATURDAY, JUNE 20, 1998, 30 OF THE 148 CARS MAKING UP EASTBOUND CSX TRAIN Q316 DERAILED AT COX LANDING, WV. OF THE DERAILED CARS, THREE WERE LOADED WITH HAZARDOUS MATERIAL, AND EIGHT OTHERS CONTAINED HAZARDOUS MATERIAL RESIDUE. TWO OF THE LOADED CARS WERE DAMAGED IN THE PILEUP AND LEAKED A COMBINED VOLUME OF ABOUT 21,500 GALLONS OF FORMALDEHYDE SOLUTION. NO ONE WAS INJURED DURING THE DERAILMENT OF THE TRAIN; HOWEVER, 15 PERSONS REPORTED MINOR INJURIES AS A RESULT OF THE RELEASE OF FORMALDEHYDE. TOTAL DAMAGES IN THE ACCIDENT EXCEEDED $2.6 MILLION.
Recommendation: NTSB RECOMMENDS THAT CABELL AND WAYNE COUNTIES' LOCAL EMERGENCY PLANNING COMMITTEE: IN COOPERATION WITH CSX TRANSPORTATION, INC., DEVELOP AND IMPLEMENT INCIDENT COORDINATION PROCEDURES THAT WILL ENSURE THAT SAFETY-CRITICAL OPERATIONS DURING WRECKAGE-CLEARING ACTIVITIES ARE COORDINATED WITH ALL PARTIES INVOLVED IN THOSE ACTIVITIES.

Response: The Safety Board is aware through correspondence with CSXT that the intent of this recommendation has been met through CSXT's Corporate Best Practices Document for Derailment Management, which assigns specific responsibilities for coordination procedures in the event of wreckage clearing activities. Accordingly, Safety Recommendation R-99-10 is classified "Closed--Acceptable Action."

Response: MS. RICHMOND'S LETTER DID NOT REFERENCE R-99-10; HOWEVER, THROUGH FOLLOW-UP CONTACT, THE BOARD HAS LEARNED THAT CSX IS NOW MEETING WITH C/WLEPC. UNITL WE HAVE RECEIVED VERIFICATION THAT THE INCIDENT COORDINATION PROCEDURES HAVE BEEN IMPLEMENTED, THE SAFETY BOARD HAS CLASSIFIED R-99-10 "OPEN--ACCEPTABLE RESPONSE."

Response: Letter Mail Controlled 10/13/1999 12:03:48 PM MC# 991127 THE ADDRESSEE DID NOT ADDRESS THIS RECOMMENDATION IN THEIR RESPONSE. HOWEVER, DURING FOLLOW-UP, SR LEARNED THAT CSX AND C/WLEPC ARE COOPERATING AND THAT THERE IS A GOAL OF HOLDING A FULL SCALE DRILL DURING 2000.
