Synopsis: ON 3/25/98, ABOUT 4:48 A.M. EASTERN STANDARD TIME, SOUTHBOUND NORFOLK SOUTHERN CORPORATION (NORFOLK SOUTHERN) TRAIN 255L5, WHICH WAS EN ROUTE TO FORT WAYNE, INDIANA, STRUCK EASTBOUND CONSOLIDATED RAIL CORP. (CONRAIL) TRAIN TV 220, WHICH WAS EN ROUTE TO COLUMBUS, OHIO. THE COLLISION OCCURRED WHERE THE NORFOLK SOUTHERN HUNTING DISTRICT AND THE CONRAIL CHICAGO MAIN LINES CROSS AT GRADE AT THE EAST END OF THE TOWN OF BUTLER, IN. BOTH LOCOMOTIVES AND FIVE CARS FROM THE NORFOLK SOUTHERN TRAIN DERAILED, AND THREE CARS FROM THE CONRAIL TRAIN, TWO WITH MULTIPLE STACKED PLATFORMS, DERAILED. THE NORFOLK SOUTHERN CONDUCTOR WAS KILLED; THE ENGINEER AND STUDENT ENGINEER SUSTAINED MINOR INJURIES. THE TWO CONRAIL CREWMEMBERS WERE NOT INJURED.
Recommendation: THE NTSB RECOMMENDS THAT NORFOLK SOUTHERN CORPORATION: INFORM ALL OPERATING PERSONNEL OF THEIR RESPONSIBILITIES REGARDING STUDENT ENGINEERS.

Response: Safety Board staff reviewed the video It's Your Job, which was included with your December 27, 2000, response to this recommendation, and found it to be an excellent tool to inform operating personnel of their responsibilities regarding student engineers. The Safety Board is pleased that you presented this video, during an 8-hour safety workshop, to 15,200 transportation employees. The Board also commends NS for including the video in the training curriculum for train service employees and for providing it to designated supervisors of locomotive engineers. Accordingly, Safety Recommendation R-99-17 is classified "Closed--Acceptable Action."

Response: Letter Mail Controlled 01/03/2001 5:15:39 PM MC# 2010002: As discussed in our response to R-99-15, the NS Operating Rules Committee concluded that Operating Rule 106 as currently written addresses the responsibilities of the student engineer. An NS video focusing on informing operating personnel of their responsibilities concerning engineer trainees has been completed. A copy of the completed training video, "It's Your Job" and leader guide is enclosed. NS has elected to enhance its original plan for this program and now use the video in a more formal training environment. This video is part of an eight-hour safety workshop currently being presented to 15,200 transportation employees. Teams of Contract and Non-Contract personnel are conducting this training, with expected completion during the fourth quarter of 2000. The video has been added to the training curriculum for train service employees at the NS Training Center, McDonough, GA., and is included in training provided to Designated Supervisors of Locomotive Engineers. NS believes with the completion of the video and training module and the current and future training plans, this brings closure to R-99-17.

Response: THE SAFETY BOARD NOTES THE NS'S INTENTION TO PRODUCE A VIDEO INFORMING OPERATING PERSONNEL OF THEIR RESPONSIBILITIES REGARDING ENGINEER TRAINEES AND REVISE OPERATING RULE 106 TO HIGHLIGHT THE RESPONSIBILITIES OF LOCOMOTIVE ENGINEER TRAINEES (LET). PENDING DISTRIBUTION OF THE VIDEO AND REVISION OF RULE 106, THE SAFETY BOARD HAS CLASSIFIED R-99-17 "OPEN--ACCEPTABLE RESPONSE."

Response: Letter Mail Controlled 10/28/1999 4:23:24 PM MC# 991193 NS HAS PROPOSED A REVISION TO OPERATING RULE 106. THIS PROPOSED REVISION WILL HIGHLIGHT THE RESPONSIBILITIES OF LET'S WHEN OPERATING A TRAIN TO BE THE SAME AS THAT OF A LOCOMOTIVE ENGINEER. EACH LET RETURNING FROM THEIR INITIAL CLASSROOM TRAINING WILL BE PLACED UNDER THE SUPERVISION OF A DISTRICT ROAD FOREMAN OF ENGINES (RFE) FOR ON-THE-JOB (OJT) TRAINING GUIDANCE. PRIOR TO OPERATING AS A STUDENT ENGINEER, THE DISTRICT RFE WILL MEET WITH THE LET FOR AN ORIENTATION TO THE OJT TRAINING. THIS ORIENTATION WILL INCLUDE AN EXPLANATION OF THE OJT TRAINING PROCESS, ASSIGNMENT TO A COACH-TRAINED ENGINEER, A SCHEDULE OF TRAINING ASSIGNMENTS AND A QUESTION AND ANSWER SESSION. TO FURTHER EMPHASIZE RESPONSIBILITIES REGARDING STUDENT ENGINEERS, NS WILL PRODUCE A VIDEO TO BE SHOWN SYSTEMWIDE TO ALL T&E PERSONNEL. RESPONSIBILITIES OF LET'S DURING ALL PHASES OF TRAINING, RESPONSIBILITIES OF COACH ENGINEERS FOR LET'S DURING TRAINING, RESPONSIBILITIES OF OTHER CREW MEMBERS, ESPECIALLY CONDUCTORS, FOR LET'S, AND RESPONSIBILITIES OF SUPERVISORY PERSONNEL, DISPATCHERS AND YARD MASTERS IN WORKING WITH LET'S.
