Synopsis: ON 3/25/98, ABOUT 4:48 A.M. EASTERN STANDARD TIME, SOUTHBOUND NORFOLK SOUTHERN CORPORATION (NORFOLK SOUTHERN) TRAIN 255L5, WHICH WAS EN ROUTE TO FORT WAYNE, INDIANA, STRUCK EASTBOUND CONSOLIDATED RAIL CORP. (CONRAIL) TRAIN TV 220, WHICH WAS EN ROUTE TO COLUMBUS, OHIO. THE COLLISION OCCURRED WHERE THE NORFOLK SOUTHERN HUNTING DISTRICT AND THE CONRAIL CHICAGO MAIN LINES CROSS AT GRADE AT THE EAST END OF THE TOWN OF BUTLER, IN. BOTH LOCOMOTIVES AND FIVE CARS FROM THE NORFOLK SOUTHERN TRAIN DERAILED, AND THREE CARS FROM THE CONRAIL TRAIN, TWO WITH MULTIPLE STACKED PLATFORMS, DERAILED. THE NORFOLK SOUTHERN CONDUCTOR WAS KILLED; THE ENGINEER AND STUDENT ENGINEER SUSTAINED MINOR INJURIES. THE TWO CONRAIL CREWMEMBERS WERE NOT INJURED.
Recommendation: THE NTSB RECOMMENDS THAT NORFOLK SOUTHERN CORPORATION: ASSIGN SUPERVISORS DEDICATED EXCLUSIVELY TO STUDENT ENGINEERS WHO WILL, AT A MINIMUM: MEET WITH STUDENT ENGINEERS AT THE START OF THE ON-THE-JOB TRAINING PHASE TO ENSURE THAT STUDENT ENGINEERS ARE AWARE OF THE CONDITIONS UNDER WHICH THEY CAN OPERATE A TRAIN AND THAT THEY KNOW WHAT TO DO IF THESE CONDITIONS ARE NOT MET; TRACK THE STUDENT ENGINEER'S DAILY TRAIN ASSIGNMENTS, DAILY CREW ASSIGNMENTS, AND PERFORMANCE EVALUATIONS; AND PROVIDE TIMELY FEEDBACK AND ADVICE TO STUDENT ENGINEERS ON A CONTINUING BASIS.

Response: THE SAFETY BOARD IS PLEASED TO LEARN THAT THE NS HAS PREPARED A SET OF INSTRUCTIONS TO BE USED BY DISTRICT ROAD FOREMEN OF ENGINES WHILE TRAINING STUDENT ENGINEERS. THE NEW GUIDELINES SHOULD PROMOTE CONSISTENT APPLICATION OF THE NS'S LET PROGRAM SYSTEMWIDE. BASED ON THE NS'S NEW GUIDELINES FOR SUPERVISORS, R-99-18 HAS BEEN CLASSIFIED "CLOSED--ACCEPTABLE ACTION."

Response: Letter Mail Controlled 10/28/1999 4:23:24 PM MC# 991193 NORFOLK SOUTHERN BELIEVES THAT THE DUTIES AND RESPONSIBILITIES OUTLINED IN R-99-18 ARE ALREADY COVERED BY CURRENT INSTRUCTIONS TO DISTRICT ROAD FOREMEN OF ENGINES (RFE) IN THE FOLLOWING MANNER: RFE'S ARE PRIMARILY RESPONSIBLE FOR ALL LOCOMOTIVE ENGINEER TRAINEES ON THEIR TERRITORY. AS SUCH, THEY ACT AS MENTORS TO EACH LET WHILE MONITORING HIS/HER PROGRESS IN TRAINING; LET'S MEET WITH THEIR RESPECTIVE DISTRICT RFE IMMEDIATELY UPON RETURNING FROM THEIR INITIAL CLASSROOM TRAINING. THE DISTRICT RFE PREVIEWS THEIR TRAINING, PROVIDES A SCHEDULE OF OJT ASSIGNMENTS, EXPLAINS HOW THEY WILL BE EVALUATED, PROVIDES A COPY OF AN EVALUATION (FORM 292) AND REVIEWS THE FUNCTION OF AND HOW TO COMPLETE A TRIP REPORT FORM; LET'S ARE PROVIDED FEEDBACK ON A DAILY BASIS BY THEIR COACH-TRAINED ENGINEERS AND/OR THEIR DISTRICT RFE'S. SUGGESTIONS AND RECOMMENDATIONS ARE PROVIDED TO LET'S TO IMPROVE SKILLS AND OVERALL PERFORMANCE. HOWEVER, AS DESCRIBED IN RESPONSE TO R-99-17, IN ORDER TO ENSURE A SYSTEMWIDE APPLICATION NS HAS PREPARED A SET OF INSTRUCTIONS TO BE USED AS A GUIDELINE BY SUPERVISORS OF STUDENT ENGINEERS.
