Synopsis: ON 3/25/98, ABOUT 4:48 A.M. EASTERN STANDARD TIME, SOUTHBOUND NORFOLK SOUTHERN CORPORATION (NORFOLK SOUTHERN) TRAIN 255L5, WHICH WAS EN ROUTE TO FORT WAYNE, INDIANA, STRUCK EASTBOUND CONSOLIDATED RAIL CORP. (CONRAIL) TRAIN TV 220, WHICH WAS EN ROUTE TO COLUMBUS, OHIO. THE COLLISION OCCURRED WHERE THE NORFOLK SOUTHERN HUNTING DISTRICT AND THE CONRAIL CHICAGO MAIN LINES CROSS AT GRADE AT THE EAST END OF THE TOWN OF BUTLER, IN. BOTH LOCOMOTIVES AND FIVE CARS FROM THE NORFOLK SOUTHERN TRAIN DERAILED, AND THREE CARS FROM THE CONRAIL TRAIN, TWO WITH MULTIPLE STACKED PLATFORMS, DERAILED. THE NORFOLK SOUTHERN CONDUCTOR WAS KILLED; THE ENGINEER AND STUDENT ENGINEER SUSTAINED MINOR INJURIES. THE TWO CONRAIL CREWMEMBERS WERE NOT INJURED.
Recommendation: THE NTSB RECOMMENDS THAT NORFOLK SOUTHERN CORPORATION: PROVIDE STUDENT ENGINEERS WITH FORMAL TRAINING IN ALL KNOWN OR ANTICIPATED OPERATIONAL REQUIREMENTS SYSTEMWIDE, INCLUDING OPERATING TRAINS WITH THE LONG HOOD FORWARD.

Response: THE SAFETY BOARD NOTES THAT THE NS MODIFIED THE CONTENT OF THE TRAINING PROGRAM TO REFLECT CURRENT OPERATING REQUIREMENTS. WE WERE ALSO PLEASED TO LEARN THAT THE NS HAS ADDED FORMAL TRAINING ON THE OPERATION OF LONG-HOOD FORWARD LOCOMOTIVES, ALONG WITH A NEW REQUIREMENT THAT TRAINEES ACTUALLY OPERATE THE LOCOMOTIVES AT THE NS TRAINING CENTER. BASED ON YOUR RESPONSE, R-99-19 HAS BEEN CLASSIFIED "CLOSED--ACCEPTABLE ACTION."

Response: Letter Mail Controlled 10/28/1999 4:23:24 PM MC# 991193 IN THE NORMAL COURSE OF ACT (A DURATION OF FOUR TO SIX MONTHS) AND TENURE AS A QUALIFIED CONDUCTOR, THEY WILL EXPERIENCE LONG-HOOD FORWARD OPERATIONS ON A FREQUENT BASIS. TRAINING CENTER PERSONNEL ARE IN FREQUENT CONTACT WITH TRANSPORTATION, OPERATING RULES AND THE SYSTEM GENERAL FOREMAN DEPARTMENTS TO INSURE ACCURACY OF SUBJECT MATTER. THE CONTENT OF THE TRAINING IS MODIFIED AS NEEDED TO REFLECT CURRENT OPERATING REQUIREMENTS. BEGINNING WITH THE LET CLASS THAT BEGAN TRAINING AT MCDONOUGH ON 8/26/99, MATERIAL WAS ADDED TO THE PROGRAM SPECIFICALLY ADDRESSING THE ISSUE OF OPERATING WITH THE LONG HOOD FORWARD. IN ADDITION, EACH TRAINEE AT MCDONOUGH IS NOW REQUIRED TO ACTUALLY OPERATE A LOCOMOTIVE WITH LONG HOOD FORWARD DURING A PRACTICE RUN ON THE TRAINING CENTER'S PRACTICE TRACK.
