Synopsis: ON 3/25/98, ABOUT 4:48 A.M. EASTERN STANDARD TIME, SOUTHBOUND NORFOLK SOUTHERN CORPORATION (NORFOLK SOUTHERN) TRAIN 255L5, WHICH WAS EN ROUTE TO FORT WAYNE, INDIANA, STRUCK EASTBOUND CONSOLIDATED RAIL CORP. (CONRAIL) TRAIN TV 220, WHICH WAS EN ROUTE TO COLUMBUS, OHIO. THE COLLISION OCCURRED WHERE THE NORFOLK SOUTHERN HUNTING DISTRICT AND THE CONRAIL CHICAGO MAIN LINES CROSS AT GRADE AT THE EAST END OF THE TOWN OF BUTLER, IN. BOTH LOCOMOTIVES AND FIVE CARS FROM THE NORFOLK SOUTHERN TRAIN DERAILED, AND THREE CARS FROM THE CONRAIL TRAIN, TWO WITH MULTIPLE STACKED PLATFORMS, DERAILED. THE NORFOLK SOUTHERN CONDUCTOR WAS KILLED; THE ENGINEER AND STUDENT ENGINEER SUSTAINED MINOR INJURIES. THE TWO CONRAIL CREWMEMBERS WERE NOT INJURED.
Recommendation: THE NTSB RECOMMENDS THAT NORFOLK SOUTHERN CORPORATION: PROVIDE EMPLOYEES, ESPECIALLY TRAINEES, WITH EFFECTIVE STRATEGIES FOR DEALING WITH CREWMEMBERS WHO KNOWINGLY DISREGARD THE OPERATING RULES.

Response: The Safety Board commends NS for producing the video For Safety's Sake...Communicate! Safety Board staff has reviewed the video and found it to be an excellent method of providing operating employees effective strategies for dealing with crewmembers who knowingly disregard operating rules. The Board is pleased that the program for enhancing communication skills is now included in the training curriculum for newly hired train service employees. Based on your response, Safety Recommendation R-99-21 is classified "Closed--Acceptable Action."

Response: Letter Mail Controlled 01/03/2001 5:15:39 PM MC# 2010002: The program for enhancing the communication skills outlined in NS' response of October 26, 1999 is complete. A copy of the video "For Safety's Sake...Communicate!" is enclosed. The module was included in our 2000 Operating Rules classes. These classes were completed during the first and second quarters of 2000. This program is now incorporated into our training curriculum for new hire train service employees at the NS Training Center, McDonough, GA NS believes, with the completion of the Operating Rules classes for year 2000 and the inclusion of the program in the Training Center curriculum, that the necessary actions to complete R-99-21 have occurred.

THE PROGRAM WILL INCLUDE CLASS DISCUSSIONS AND ROLE-PLAYING PRACTICE SESSIONS BASED ON REALISTIC RAILROAD SCENARIOS INCORPORATING COMMUNICATION STRATEGIES FOR EFFECTIVELY DEALING WITH EXPERIENCED PEERS OR SUPERIORS. SO THAT ALL TRAIN SERVICE EMPLOYEES RECEIVE AN OVERVIEW OF THIS MATERIAL, A SEGMENT OF THE YEAR 2000 OPERATING RULES CLASSES WILL INCORPORATE THIS MATERIAL.
